Polaris Healthcare and Rehabilitation Center - Social Services PRN Skilled Nursing FacilityPolaris Healthcare and Rehabilitation Center is seeking an experienced, caring person to join the team! Polaris is a beautiful nursing facility with warm and friendly staff that provides excellent care to our residents!The purpose of this position is to provide services to meet the social and/or emotional needs that affect the residents' ability to achieve their highest level of function; participate in the development of resident's comprehensive care plans; develop policies/procedures; and provide social services to residents in compliance with federal, state and local regulations.
Responsibilities include:
Develop and implement social service policies and procedures to meet residents' needs in compliance with federal, state and local regulations.
Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
Develop one-to-one professional relationships with residents and families as needed for counseling.
Counsel residents and families in dealing with feelings about death or dying and other emotional, mental environmental or physical limitations Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately Document the social service component of the Comprehensive Care Plan for each resident in a timely manner Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.
Assess, plan and document residents' discharge needs in accordance with facility discharge planning policy and regulatory requirements.
Coordinate family and community resources as needed to solve financial needs and to promote emotional security.

Qualifications:
BSW required MSW a plus.
Excellent communication skills and the ability to interact with all personalities Ability to build strong relationships and gain support from staff.
Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care Previous long-term care experience is desirable.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
